That pattern is moisture in the cavity between the cover and the lens, which means water is getting in past the seat and warming air is driving it off once the camera heats up. Pull the cover, dry both faces and the lens completely, inspect the seat and any gasket for damage or compression set, and refit. If it fogs again after that, the cover is no longer sealing and should be replaced.

Should I leave the protective film on during the season for extra protection?

No. Peel film is packaging, not a service coating. It is not optically flat, it hazes and lifts at the edges in the sun, and it traps water underneath when it starts to release, so it degrades the picture rather than saving the cover. Peel it as you fit the cover. If you want a sacrificial layer, the cover itself is the sacrificial layer, which is the whole reason it is a separate replaceable part.

My picture is dark and washed out, but the cover looks clean. Is the cover still the suspect?

Test it rather than assume. Point the nose at a strong light and see whether the image collapses, then wet the surface and look again. Fine even abrasion is nearly invisible on a clean cover and still eats a large amount of contrast. If the picture is unchanged into the light and unchanged wet, the cover is not the cause and you should be looking at display brightness, camera settings, or the camera module itself.

Release the cover, check that nothing is trapped under the edge, confirm the seat groove is clear of packed dirt, and bring it down evenly across the whole perimeter instead of pressing the last clip home from the center of the face.

How do I know whether the haze is on the cover or on the lens itself?

Compare the two faces separately. With the cover off, clean it and hold it up to a light with the flashlight raking across it, then look at the lens on its own under good light. Damage on the cover shows as scratches, bloom, or crazing you can see in the material. Damage on the lens usually shows as a matte patch or a ring of circular scuffing where a loose cover has been working against it. Do this before ordering, since the fix is different for each.
